Permanent Supportive Housing Case Manager

Maintain a caseload of approximately 15 very low-income seniors who have experienced chronic homelessness. Work collaboratively with seniors to develop effective housing plans to address immediate and long-term housing and healthcare needs, using a strengths-based and client-centered approach. This is a full-time position, earning $42,663 annually and excellent benefits including medical and dental insurance, four weeks of paid vacation per year, Employee Assistance Program, disability and life insurance, a mission-driven and supportive work environment.

Bilingual Housing Case Manager

Maintain a caseload of approximately 30 very low-income seniors who are experiencing housing instability, with a focus on Spanish-speaking seniors. Provide coordination and assistance with housing applications, advocacy, resources and services that support clients and their housing plans. This is a full-time position, earning $44,098 annually and excellent benefits including medical and dental insurance, four weeks of paid vacation per year, Employee Assistance Program, disability and life insurance, a mission-driven and supportive work environment.
